Forms & e-signing

Kill the clipboard
at reception

Build intake and consent forms, send them as a link, get them back signed. No printing, no retyping, and no account for the patient to create.

The slowest part of a first visit happens before anyone sees a clinician.

Send the form when the appointment is booked and the patient walks in already registered.

Build a form by dragging blocks

Questions, headings, signature fields. Reorder by dragging. No developer, no PDF layout tool, no three-week wait on a supplier.

Keep the PDFs you've already got

Upload an existing fillable PDF and patients complete it in the browser. Your consent forms don't need rebuilding to be usable.

Signed with a finger

Signature fields work on whatever device the patient is holding, and the signature is stored with the completed form.

No login for the patient

Forms open from a secure link and submit back to the practice. One less password standing between a patient and their appointment.

Intake, consent and clinical forms in one library

Templates sit in categories and get reused across the practice. Every completed form is filed against the patient and the visit it belongs to.

How it works

What happens inside Saya, from the first click to the patient record.

  1. 01

    Preview what the patient sees

The builder shows the respondent view as you edit, including changes you haven't saved. You catch the confusing question before 40 people answer it.

  2. 02

    Send from wherever you are

Forms go out over WhatsApp, SMS or email from the patient or appointment you already have open, and land in the same communications log.

  3. 03

Patients fill in their own file

Profile intake asks new patients for their demographics, contact details, medical aid membership, allergies, medications and history before they arrive.

  4. 04

    Print it when someone asks

Every completed form renders to a PDF with the answers and signatures on it, for referrals, records, or the patient who wants a copy.
